The Minnesota Secretary of State's office provides copies of UCC & Tax Liens filed with our office.  The fee for UCC and Notice of Tax Lien copies is $1.00 per page.

To request a copy of a UCC or Notice of Tax Lien record you may submit a written request providing the name of the debtor for which you are requesting information and the file number of the UCC document (if known), or if you are a Direct Access Subscriber Order Online.

Standard forms are provided for a Debtor Name Search or Copy Request, Copy by Number, Secured Party List Request, or UCC Bulk Order Forms:

• UCC-11 National Form  
• UCC-12 Request for Tax Lien Information or Copies
• UCC Secured Party List Request Form

• UCC Bulk Order Form
